Case report: Catastrophic event: neonatal gastric perforation and complication of capillary leak syndrome

Neonatal gastric perforation (NGP) is a rare, but life-threatening condition that can lead to serious conditions, such as capillary leak syndrome (CLS). Here, we present the case of preterm male infant with NGP complicated by CLS after stomach repair. The patient was born at 33 2/7 weeks, weighed 1,770 g, and diagnosed respiratory distress syndrome. On fourth day life, presented distention an unstable cardiovascular system. Routine blood tests revealed white cell count 2.4 × 10 9 /L. Chest abdominal radiography pneumoperitoneum, suggesting gastrointestinal perforation. urgently transferred tertiary hospital for exploratory laparotomy, where 2 cm diameter discovered in wall subsequently repaired. Pathological findings indicated absence muscular layer wall. unexpectedly developed postoperatively, leading multiorgan dysfunction eventual death. underlying pathological mechanism NGP-induced may be related severe chemical peritonitis, sepsis, endothelial glycocalyx dysfunction, enhanced systemic inflammation, translocation gut microbiota, causing hyperpermeability. Notablely, surgery itself significant triggering factor occurrence. Complications are extremely dangerous. Investigating which triggers could potentially improve prognosis. Conservative treatment pneumoperitoneum secondary reasonable option, especially when unstable.
